Member card example

On this page you find an example of a member card that also serves as a blood group card. You enter the data for this yourself when you register as a member, and you can adjust it (e.g. the emergency information) in your account at any time.
This ID card also contains a short form of your living will.
You can print the ID card, at home, at the office, from an online vendor, or at a copy store. On paper (print, fold and laminate) or on a plastic card.
But you can also use it digitally, for example, download it to your smartphone and save it there in your photo album or Apple Wallet or similar.
Additionally, you can import both member ID and living will directly into the emergency information in your smartphone, which emergency medical personnel can retrieve without having to unlock the phone.
The QR code and link above is also for emergency medical personnel. scanning this code/link will take them directly to your living will and all relevant medical data, in a special version only for professionals, who will find there the option to translate this data into the main world languages in a few seconds.

The “View living will” button allows you to view your living will. Before you can do that, however, you must first edit and save it (you will find a template that you can customize as you wish).
